The April workshop on Advanced Healthcare Directives is unfortunately cancelled. However, all adults (no matter your age) should have a current health care directive. Do you? If you do not, now may be a good time. If you are home, you can do your homework. Having an advanced directive allows you to take charge of your health care decisions and put the worry of your loved ones aside. Here are a couple of suggestions to get you started:
The advanced directive used in the St. Pius X workshop is called “Honoring Choices Wisconsin”. It is an initiative of the Wisconsin Medical Society. The document has good information and is user friendly. Click here for the Advanced Directive form. A second option provides additional types: The Wisconsin Department of Health Services (DHS) has advanced directive forms (for health and finance) and information available here. The forms available are: Declaration to Physicians (Wisconsin Living Will), Power of Attorney for Health Care, Power of Attorney for Finance and Property and Authorization for Final Disposition.
